Informed Consent for Telehealth Services

By engaging with Meddy's telehealth services, you consent to receiving medical consultations and, where clinically appropriate, treatment via electronic communications including video, phone, and asynchronous messaging.

Nature of Telehealth

Telehealth involves the delivery of healthcare services using electronic communications. It differs from in-person care in that the provider cannot physically examine you. You understand and acknowledge:

• The telehealth provider will rely on information you provide to make clinical decisions
• Technical failures could interrupt or prevent the delivery of services
• Telehealth may not be appropriate for all medical conditions
• In a medical emergency, you should call 911 or visit the nearest emergency room

Provider Independence

All healthcare providers who deliver services through the Meddy platform are independent licensed professionals. They exercise sole clinical judgment and are solely responsible for the care they provide. Meddy does not direct, supervise, or control clinical decisions.

Compounded Medications

If your provider prescribes a compounded medication, you acknowledge:

• Compounded medications are not FDA-approved
• They are prepared by licensed compounding pharmacies in FDA-regulated facilities
• Risks may differ from commercially manufactured products
• You have the right to discuss all risks and alternatives with your provider before proceeding

Information Sharing

You consent to Meddy sharing your health information with affiliated healthcare providers and licensed pharmacy partners as necessary to deliver your care and fulfill prescriptions.

Your Rights

You have the right to:

• Withhold or withdraw consent to telehealth at any time without affecting your right to future care
• Request a summary of your telehealth visit
• Receive information about alternative care options
• Have your health information protected in accordance with HIPAA
